PROTOCOL TO BE FOLLOWED BY PATIENT

Registration:

All patients using the services of 1HBAS are assigned a unique identification number. This is allotted to all newly registered patients and is carried forward for all services, as well as for future reference.

New Registration:

  A. Department of Psychiatry

All new and follow-up patients are registered at the registration counter. They are allotted walk- in-clinic number (UID Number, which is permanent) and seen in room no. 4, 12 and 14 by Psychiatry Senior Residents and consultants if required and treatment started.

B. Department of Neurology

New and follow-up patients of Neurology department are also registered at the registration counter and allotted UID Number, which is permanent. They are seen in room nos. 18 to 21. 24 to 26 by consultants & Senior Residents.

       C. Department of Neurosurgery

All new patients are registered at the registration counter and allotted UID Number, which is permanent and are subsequently seen in room number 23. There is no policy to register patients under department of Clinical Psychology or Psychiatry Social Work. Patients are registered under Departments of Psychiatry, Neurology and Neurosurgery before they are referred to department of Clinical Psychology/ Psychiatry Social Work and other section services for e.g. physiotherapy, occupational therapy etc for assessment as well as interventions.

Follow-Up Cases:

All cases are sent to their respective departments/ rooms after registration the cases are reassessed by the concerned specialists and appropriate treatment is continued.

Detailed work-up

Detailed work-up and discussion is warranted in psychiatric patients for a detailed evaluation and management at initial visit or review of diagnosis and management at a later point of time if it is required. In Neurology and Consultants in subsequent visits.

Registration timings:

IHBAS has a token display system for the patient registration as well as medicine distribution. The token distribution timing is 7:30am to 9:30am everyday except Sunday & Gazetted holidays :

The registration timing is 8.00 AM to till last token for Morning OPD and 1.30 PM To till last token for Specialty clinics on all working days.

Hospital charges:

Amount of Rs. 10 as registration fee for all patients both new as well as old and for each follow-up visit is charged. Except Senior citizen handicapped & homeless Poor patients, patients brought by police or the patients referred by NGOs are exempted from the user charges by MS/DMS, following assessment by the Psychiatric Social Worker on duty and recommendation by the unit consultants.

Registration Counters:

There are currently 11 registration counters working. These includes 5 counters for all new cases and 6 counters for all old cases including a separate registration counter for elderly, freedom fighters, staff patients, physically challenged patients and jail referred cases. One counter is there for allotting work cases to be worked up on a given day.

Facility for Senior Citizens and Freedom Fighters:

Senior citizens and freedom fighters are seen on top priority.

PROCEDURES

1. OPD Registration Procedure:

Patient visits the registration facility in OPD section Patients visiting the hospital are registered at the registration counter and are guided to go to room numbers 4 to 10 walk in clinic and 11, 12 & 14 for follow-up. in Psychiatry or 18, 20, 24 and 26 for Neurology and 23 for Neurosurgery.

Process of registration:

New Patient furnishes demographic details at the registration counter

There is a separate counter for the senior citizens. A patient/family member pays the registration Charges for the patients admitted in the private wards are collected by the sister- in-charge and are submitted to accounts section.

Service bill/s- is given to the patient or to family members for other hospital expenses.

At the end of the day staff on duty takes out cash records and the amount so collected are deposited in the account section of the institute.

* There is no user charge for general patient. They are treated free of cost.

A New Patient is given a fresh OPD card with OPD room number indicated. Follow up patient's OPD card is taken and the current date is entered after registration.

For Follow up patients, in case there is no space left for prescription writing on the previous OPD card, a fresh OPD card with the current date is issued.

The case files of both new as well as follow-up patients along with the OPD cards is taken to the respective OPD rooms by the OPD attendants. The same attendant keeps a record of all the files (separate register and record handling for jail referred cases and potential medico-legal cases) taken by him to that particular room and returns back the files to the registration counter as per the records made by him.
